NOVA LIFE 2015 RATES
As a courtesy and for your convenience this document has been put together to ensure that all patients have clarity on the way the consultations are run and the costs involved before their appointment.
The first consultation:
Your initial consultation is approximately 60min, during which time a full assessment is done, which includes:

ï‚· Family history of disease (genetic history) looking at predisposing factors that may precipitate undesirable weight and health consequences.

ï‚· Detailed diet history looking at food preferences, food patterns, food likes and dislikes, food allergies and intolerances, food cravings and patterns of eating behaviour.

ï‚· Health and nutritional status.

ï‚· Weight, height and measurements.

ï‚· Vitamin and Mineral supplementation review.

ï‚· If there are any blood tests required, I will discuss this with you.
Following this assessment, you will have 30 minutes of nutrition education. Education aims to help you understand:

ï‚· Food classification.

ï‚· The metabolic function of food.
-
ï‚· The impact that certain food choices can have on appetite and on fat burning capabilities.

ï‚· Any information relating to disease conditions that may require addressing.

The nutrition education is also tailored to help you understand how certain food choices can have an impact on other health problems specific to the individual.
Following the first consultation, I will analyse your diet history; and calculate an individualized meal plan and cycle menu. This is completed between the first and second consultations. The cost of this consultation is R650.
Second consultation:
The second consultation will be approximately a week after the first consultation. The duration of this consultation will be approximately 30 - 45 minutes. The session will cover further nutrition education specific to your requirements. The individually formulated menu plan and cycle menu will be presented and explained in detail. I will also advise you on any supplements you may require and help you to set goals for the attainment of health and weight objectives. The cost of this consultation will be R350.
Other follow-up consultations:
The frequency of your follow-up consultations is variable depending on your needs. This will be discussed with you. Research has shown that people who are followed-up on a regular basis are more successful in weight and health management than those who are not. Please enquire about Nova Life packages available and current special offers.
All subsequent follow-up consultation are billed per time e.g. 1 unit = 20 minutes = R280.00. If extra time is required during your consultations, please note that you will be billed accordingly. Research has also suggested that this follow-up/accountability is most crucial in the first 6 months after embarking on lifestyle and dietary intervention.
Online support
Ongoing online support available at an additional R350.00 per month. Terms and conditions apply.

Payment
This is a private practice is not contracted to medical aids, which means that we require you to settle your account before each appointment. Internet Banking payments need to be made prior to the appointment (Banking details: Account name: A. Steyn, Capitec Bank (savings account), Account Number: 1363 880 630, Branch: 470010, Ref: Your Full Name).

Cancellation of appointments:
There is a 24 hour-cancellation policy i.e. we request that you cancel appointments at least 24 hours in advance. This also applies to the cancellation or rescheduling of appointments. Appointments not cancelled timeously will be charged a cancellation fee of R180. Appointments not kept and not cancelled will be charged the full rate. Thank you for your understanding in this regard. You can be assured that your times and appointments will be facilitated as far as it is possible.
Please bring along any supplements or medications you are currently taking so that they can be assessed on what should be added or removed from your regimen.
